What states do you practice in?

Currently, residents of Oregon or individuals who are physically in the state during services can book in-person visits or telepsychiatry appointments with us. However, we hope to expand our services to the broader community, so please continue to check back as we add additional states to our service area.

What are your hours of operation?

Telepsychiatry appointment hours:  Monday- Friday 05:30-1800.  Saturday – Sunday from 06:00 -11:00.
We typically schedule in-person office appointments on weekends due to the practitioner’s availability. If you have any questions or concerns about office hours, or if you do not see an open appointment slot, please call or text us, as we may be able to accommodate you.

What ages and patient types do you treat?

Amity offers mental health care services for individuals aged 18 to 65 but may see adolescents on a case-by-case basis.

We treat a variety of mental health conditions, including but not limited to:

  • Adult Attention Deficit Hyperactivity Disorder (ADHD)
  • Depression
  • Anxiety
  • Post-traumatic stress disorder (PTSD)
  • Psychotic disorders
  • Mood disorders
  • Obsessive Compulsive Disorder
  • And more

What type of patients you don’t treat?

Amity Mental Wellness is a small outpatient private practice. Unfortunately, we currently lack the resources and staffing necessary to safely treat and address the following complex situations…

  • Alzheimer’s
  • Dementia
  • Developmental Disorders
  • Autism Spectrum Disorder
  • Learning Disabilities
  • Chronic Suicidality
  • Chronic Self-Harm
  • Individuals needing pain management services requiring opioids
  • **Individuals who have active or recent substance use disorders that require suboxone, methadone, or similar therapies.
  • Individuals who are seeking Ketamine treatment.

Do you perform talk therapy?

Currently, we do not offer appointments exclusively for psychotherapy, but we do incorporate elements of common psychotherapy modalities such as cognitive behavioral therapy and motivational interviewing into our services. And you may see this context reflected as a therapy billing code on your appointment explanation of benefits.

Do you accept insurance?

Yes, Amity accepts most major commercial insurance, including Aetna, Cigna, MODA, Regence, Pacific Source, Providence, and Optum.

If Amity is not credentialed with your insurance carrier, kindly consult your insurance company to determine if you have out-of-network benefits. Our licensed provider would be pleased to provide you with a receipt or superbill that you may submit to your insurance provider for reimbursement.

Do you accept self-pay?

Yes, but prior to making an appointment, we will require a form of payment on file.

For self-paying clients, we accept all major credit and debit card payments. Unfortunately, we do not accept checks. Payment is due in full at the time of service. We will not issue refunds once we have rendered the service.

Self-pay fees & other charges

  • Initial Visit (60 min): $275
  • Follow Up Visits (20 min): $140
  • Cancellation less than 24 hours before your appointment time: $75
  • No-Show Fee: $75
  • Late arrival greater than 15 minutes may result in cancellation as a No-Show
  • Document completion (e.g., FMLA, etc.): $40 per document
  • Early medication refill (NON-CONTROLLED MEDICATIONS ONLY): $20 fee

Do you offer emergency visits/crisis services?

We are a small private outpatient practice, and, unfortunately, we are unable to accommodate emergency visits for new or established clients. In this case, we recommend that you reach out to a crisis resource or visit your local emergency department. We kindly ask that you keep this in mind when reaching out to us as a new client. We appreciate your understanding as we strive to schedule your appointment at our earliest convenience.

Can you refill my medications prescribed by my family doctor/specialist?

Unfortunately, we are unable to refill medications that have been prescribed by your other medical providers.

Do you fill out paperwork for FMLA, workers compensation, or short-term disability?

If you are a client who has been seen regularly for at least six (6) months, we can discuss FMLA paperwork if your situation requires it. We do not fill out paperwork for short-term disability or workers’ compensation.

  • An appointment is required to complete FMLA documentation.
  • A $40 fee is required per document.
